This nurse-led project aims to leverage the insights and lived experiences of carers in the prevention and management of delirium in the hospitalised older patient to validate the effectiveness of PREDICT. PREDICT includes a co-designed, interactive, web-based Delirium Toolkit, that provides nurses and carers with information on delirium risk factors, therapeutic and preventive strategies and non-pharmacological interventions to re-orientate older adults who experience delirium. It also includes carer-directed support packages that consist of counselling, social prescriptions and peer-support.
The PREDICT Model of Care that this project will trial is going to provide family carers with practical resources and tools to empower them to prevent, identify and manage delirium. Enabling family carers to participate actively in the care of their loved ones, will realise significant benefits that include:
• For older people at risk of delirium in hospital, safer and higher quality care including accurate risk assessment, reduction in incidences of delirium and reduced length of stay in hospital.
• For family carers, enhanced delirium literacy and greater confidence, skills and knowledge to care for their loved one and participate in discussions with their care team.
• For nurses, stronger capacity to integrate, educate and support family carers in identifying and managing delirium.
