Evaluation of the Northside Community Service CHANGE program

The team will be responsible for finalising the Northside Community Service CHANGE Program evaluation report. The evaluation report will include:
• a 1 page summary of the main messages;
• a 3 page executive summary of the program and outcomes;
• a main body section which includes:
a) Description of the program
b) The program’s aims and objectives;
c) How it was evaluated for process and impact (i.e. methods and key evaluation questions);
d) What the evaluation found (i.e. Results);
e) A discussion of the learnings and conclusions from the program
f) Appendices including evaluation tools and data, as relevant.
