Description
The aim of this project is to improve the hospital discharge processes for older people with dementia.
The project will concentrate on engagement and partnership between older people with dementia, their carers and hospital staff. The carer-inclusive discharge process will be implemented and evaluated in one ward in each of three Queensland hospital and health services: Gold Coast Health, Cairns and Hinterland Hospital and Health Service, Townsville Hospital and Health Service.
