Description
To assess whether on-site pharmacists in residential aged care facilities (RACFs) as part of the care team, improves the appropriateness of medication use.
Primary objective: To evaluate if the new model of care – on-site pharmacists employed in RACFs- improves the appropriateness of prescribing and medication use- as determined by validated measures of Potential Inappropriate Medications (PIM).
Secondary objective: To evaluate if the new model of care reduces hospital presentations and improves other quality use of medicine indicators in RACFs
