Description

The University will provide digital health capacity building activities for up to two ACT Residential Aged Care Homes (RACHs) and their resident’s health services to support the implementation of Telehealth to enable residents access to prompt and flexible primary health care. As part of this service, the University will evaluate the project including the care model and support required, impact, lessons learned and recommendations for successful implementation in other ACT based RACH/health service partnership settings and prepare joint publications on the findings of the Residential Aged Care Telehealth Program.
