Abstract

Mrs MJ (83 years) was referred for a Home Medicines Review because she was using multiple medications. Mrs MJ's medical history included: atrial fibrillation, chronic kidney disease (stage 3; eGFR 60 ml/min/1.73 m2), GORD, vitamin B12 deficiency, hypertension, hypercholesterolaemia, hyperhomocysteinaemia, hypothyroidism, myocardial infarction (two in the 1970s), transient ischaemic attack, and analgesic nephropathy.
