Abstract
Mrs VC is an active 76-year-old lady who lives alone in her own home with her cat. She was referred for a home medicines review by her GP with the almost generic reason of 'currently taking five or more medications' and the therapeutic goal of 'medication understanding, interactions, etc'. Mrs VC has no known drug allergies and her medical history and current medications are listed below.
